If you are unable to fill out these forms online and cannot print them, you may request a printed copy from the secretary prior to the rodeo you plan to attend.
All forms will need to be filled out in full, and given to the current MKYRA Secretary.
REQUIRED: CONTESTANT/NON CONTESTANT MEMBERSHIP FORM & RELEASE AND HOLD HARMLESS LIABILITY WAIVER
Please note: Any form that needs to be notarized will need to be filled out PRIOR to the day of the rodeo you plan to attend.
- 2026 Online Membership Form (filled out and submitted on jotforms.com) You will still have to fill out the Release of Liability waiver and have it notarized.
